Please leave the following information:

1.  Your full name and contact information

2.  Who you wish us to pray for, and the connection that person has to you.

3.  What the specific prayer you wish for is.

4.  If you wish for a call from a pastor.

A reminder that we keep names on the Prayer List for three weeks, unless you ask to keep the name on longer.
